(b) Testing results with fractional ppb readings of 0.5 ppb or greater...(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement was not met as evidence by: Based on observation, interview, record review, the licensee did not comply with the section cited above in that 4 faucets at the facility tested with a lead exceedance with values of 5.5 ppb or greater which poses a potential health, safety or personal rights risk to persons in care.
Written Directives for Lead Testing (b) All licensed Child Care Centers operating in buildings constructed before January 1, 2010, shall test their water for lead pursuant to these written directives. This requirement is not met as evidenced by: Based on observation, interview, record review, the licensee did not comply with the section cited above in that the facility did not complete the Lead Water Testing by the due date which poses a potential health, safety or personal rights risk to persons in care.
(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (2)Request a transfer of a criminal record clearance. This requirement is not met as evidenced by: S1 was not currently associated to the facility due to transfer of criminal record clearance not have been done.
