101212(d)(1)(2)(A) Reporting Requirements-Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.Title 17, Section 2500, lists the following reportable communicable diseases as of March 21, 1997:..... OUTBREAKS of ANY DISEASE (Including diseases not listed in Section 2500). The director Shanelle will submit a Licensing form LIC 624 (Unusual Incident) to the Department by 11/4/25. The director will ensure that the Department is informed when there are cases of Hand, Foot and Mouth or any other communicable disease. The staff cleaned and sanitized the facility during this time frame. This is evidence by: there were at least six children that had Hand, Foot and Mouth Disease. This is considered a communicable disease and were not reported to CCL. This is a potential risk to the health and safety of children in care.
