(a) The licensee shall provide care and supervision as necessary to meet the children's needs. (1)No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement is not met as evidenced by: Based on interviews and observations, staff do not visually observe children in the bathroom at all times. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on interviews and observation, staff interferes with children's eating by using intimidation and other actions of a punitive nature. The licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Per statements obtained, staff speaks to children in a harsh, strong, firm tone, which poses a potential health, safety or personal rights risk to persons in care
General Provisions and Definitions (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement is not met as evidenced by: Based on record review, staff did not have their immunization records in the file during the inspection, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

An aide who is 18 years of age or older, and who meets the requirements of this section and Section 101216, may escort and/or assist children in going to the bathroom, and may supervise napping children as specified in Section 101230(c)(1), without being under the direct supervision of a teacher. This requirement is not met as evidenced by: Interviews with staff revealed that Staff #1 was hired as a Pre-K teacher however is sometimes left alone with children when Staff #2 must step out of the classroom to use the restroom. This poses a potential risk to the health and safety of children in care.
Buildings and Grounds. Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children. This requirement is not met as evidenced by: On 10/12/2022, LPA observed Clorox bleach bottles easily accessible to day care children which were stored under the kitchen sink cabinet. This poses a potential risk to the health and safety of children in care.
