101229 (a)(1) ) Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs.(1) No child(ren) shall be left without the supervision ... visual observation...This requirement was not met as evidence by: Based on interview, observation, and record review, C1 was playing on the play structure monkey bars when they fell off the bars into the woodchips. Staff S1 & S2 were not in close proximity, they did not have visual observation of C1 and did not witness C1 fall. S1 & S2 learned of the injury after C2 & C3 brought C1 to them which is a potential health, safety, and personal rights risk to the persons in care.
