Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1), a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) shall be submitted to the Department within seven days following the occurrence of such event. This requirement is not being met as evidenced by: On 01/09/2026 , LPA Dowling discovered that an incident involving outbreak of hand, foot and mouth was not reported to Community Care Licensing (CCL), as required by Title 22 Regulations. The incident occurred on 10/27/2025 and involved a report of outbreak of Hand, Foot and Mouth Disease by preschool children. 1. Based on the nature of the incident, it should have been reported to CCL in accordance with Title 22 Regulations Section 101212 / Reporting Requirements. This poses a potential health, safety or personal rights risk to persons in care.
