Personal Rights - (a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe… This requirement was not met as evidenced by facility out of ratio. The licensee failed to ensure proper infant - teacher ratio to provide safe environment. This poses an immediate risk to the health, safety, or personal rights of children in care. Witnesses, Staff #1 & Staff #2 (S1 & S2), revealed that while supervision was in placed, Child #1 (C1) sustained a head injury when he fell backwards from a sitting position (the incident occurred in the playground) due to teacher - child ratio (3 teachers to 13 infants).
