Administration of Child Day Care Licensing (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. This requirement is not met as evidenced by: Based on record review, the center did not comply with the section cited above in 1 out of 3 staff had a valid mandated reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Activities (a)(3):All infants shall be given the opportunity to sleep without distraction or disturbance... (C)An infant shall not be swaddled while in care. This requirement was not met as evidence by: Based on LPA observations, the licensee did not comply with the section cited as LPA observed an infant sleeping with a sleep sack which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Personal Services 101428(b) The infant shall be kept clean and dry at all times.(2) Each infant's clothing and diapers shall be changed as often as necessary to ensure that the infant is clean and dry at all times. This requirement was not met as evidence by: Based on interviews and record review, the licensee did not comply with the section cited above as some staff members indicated diaper checks are done every 2 hours which poses/posed a potential health, safety or personal rights risk to persons in care.
(a) A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation.This requirement was not met as evidenced by the following: CCC failed to adhere to the terms of a 10/31/22 waiver issued by CCLD for the staggered use of an outdoor play area in that CCC exceed capacity of four children in the area. This poses at potential risk to the health, safety and personal rights of persons in care.
