This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPAs observed spider webs on the equipment, a broken piece of wood along the fence and a jagged piece of plastic along another fence which poses/posed a potential health, safety or personal rights risk to persons in care.
