Personnel Requirements At least one staff member who is trained in pediatric cardiopulmonary resuscitation and pediatric first aid... shall be present when children are at the child care center or offsite for center activities. Sign In and Sign Out the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the child care center...The person who signs the child in/out shall use his/her full legal signature and shall record the time of day. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above, in that sign in sheets were missing signatures and times for children being signed in and or out which poses/posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance Based on record review, the licensee did not ensure all staff was working, at the facility without a background clearance transfer ... which poses an potential Health and Safety risk to persons in care. This requirement is not met by evidence by: Staff Julya Baca was found to have been working, residing or volunteering at the facility without a clearance transfer. Date of hire was 6/1/23.
(c) The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions. This requirement is not met as evidenced by: Based on observation, interview, record review, the licensee did not comply with the section cited above facility is using the room #1,#2,#3, #4 (classrooms located by the office room and addition classroom by the swimming pool for the ealry interventation program. Per Assistant Director, these classrooms is licensed from Dept. Social Services which poses/posed a potential health, safety or personal rights risk to persons in care.
