The licensee shall be present in the home and shall ensure that children in care are supervised at all times...This requirement was not met as evidenced by: Based on interviews, staff were aware that children had a history of overcrowding on the rotating seesaw, but did not take active or appropriate steps to stop the behavior to prevent a foreseeable injury creating an environment for a lack of supervision.

(i) If an infant falls asleep before being placed in a crib or play yard, the provider shall move the infant to a crib or play yard as soon as possible. This requirement was not met as evidenced by: based on observation and interview, an infant in care has fallen alseep in a feeding chair that was placed on couch without any safety restraints. The Licensee also has not documented sleep checks for the infant after the child reached twelve months of age which poses a potential health, safety or personal rights risk to persons in care.
