Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). This regulation was not met by: On March 19, 2024, a child was unsupervised in classroom (26) for approximately one (1) minute. Based on a record review that was self-reported via an unusual Incident report dated March 25, 2024, facility staff failed to maintain 100% supervision of C1. This posed an immediate risk to the health, safety, and personal rights of the child in care.
