(1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement was not met as evidenced by: Based on the observation, interview, and record review, on 2/8/2023, the facility did not properly supervise Child 1 and Child 2 which resulted in inappropriate contact between the children which poses an immediate health, safety, or personal rights risk to persons in care. A $500 civil penalty is being assessed as a result of neglect and lack of supervision.
