If testing indicates an Action Level Exceedance at any water outlet, the water from that outlet is deemed not safe to drink and an immediate response pursuant to section 101704 shall be required. This requirement was not met as evidenced by results from the water lead sampling test showing an ALE of 6.6 ppb in water source (G). This poses a potential risk to the health, safey, and/or personal rights of children in care.

Responsibility for Providing Care and Supervision: No child shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by interviews with staff conducted during today’s inspection/. Staff stated a child was left in outside play area alone for less than two (2) minutes. This poses an immediate risk to the health, safety or personal rights of children in care.
