101229(a) The licensee shall provide care and supervision... No child(ren) shall be left without the supervision of a teacher at any time... Supervision shall include visual observation. This requirement is not met as evidenced by: Based on record review, interviews and video footage, the facility staff did not comply with the section cited above, as a child was left unsupervised for 8 minutes in the facility restroom during the transition to the playground. A teacher from another classroom found the child unsupervised, which poses an immediate health, safety, and personal rights risk to persons in care.
