Safety precautions shall include but not limited to: This requirement is not met as evidenced by: Based on Liciensing Program Analsyts (LPAs) Anita Tristan and Pa Kou Vue's observation, the licensee did not comply with the section cited above in LPAs obeserved in the Licensee's backyard which is accessible to children, power tools such as tree trimmer, grass trimmer and leaf blower. In addition, a broken fence board located behind the play yard equipment.

Infant Safe Sleep An Individual Infant Sleeping Plan [LIC 9227 (3/20)] shall be completed for each infant up to 12 months of age the provider has in care and included in the infant's file at the facility. This requirement is not met as evidenced by: Based on Liciensing Program Analsyts (LPAs) Anita Tristan and Pa Kou Vue's observation, the licensee did not comply with the section cited above. Licensee had two infants in care (infant #1 DOB: 08/22/2022 and infant #2 DOB: 07/26/2022). Licensee was not able to readily produce Infant Safe Sleep log per 102425 Infant Safe Sleep (D) Documentation shall be maintained in the infant’s file and be available to the Department for review. Documentation shall include the following: a. Date. b. Infant’s name. c. Time of each 15-minute check.
