This requirement was not met evidenced by: Based on interviews and document review, Child #1 sustained an injury while in care that required medical attention. Per interviews, child #1 was hanging from the soccer goal nets and the goal posts flipped and landed on the child. Per interviews, the goal posts were not secured and anchored to the ground during the incident, which posed an immediate risk to the health, safety, and personal rights of children in care.
