During the investigation, LPA conducted interviews, and it was confirmed that staff member yelled at and handled a daycare child in a rough manner. Based on interviews, the licensee did not comply with the section cited above. During the case management inspection, it was confirmed that staff members failed to ensure that a daycare child was released to the right authorized representative. As staff discovered that the child was released to the wrong authorized representative, staff went after the authorized representative and observed that the child was already inside the authorized representative’s vehicle. This poses an immediate risk to the health, safety, or personal rights risk to persons in care.

During the interview, it was disclosed that a staff member observed a child who was crying, upset and child disclosed that child bump child’s head. The incident was not reported to the designated individuals. It was discovered that child sustained another injury result from the fall that child bumped child’s head.
