- REPORTING REQUIREMENTS - Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Dept. by telephone or fax within the Dept. next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. Events reported shall include the following: Any unusual incident or child absence that threatens the physical or emotional health or safety of any child. This requirement was not met as evidenced by staff interviews. The investigation revealed that on 8/13/2021 mold was found in the infant classroom. Staff failed to notify CCL. Additionally, authorized representatives were not notified in accordance with Title 22 regulation 101212(f). This poses a potential risk to the health, safety or personal rights to children in care.

- SIGN IN AND OUT - Notwithstanding Section 101226.1(b)(2)(A), and in addition to Section 101229.1, the following shall apply: The licensee shall require that each child be signed in and out by his/her authorized representative. During the COVID-19 pandemic Staff were signing children in/out from the center which is the responsibility of only the authorized representative. This poses a potential risk to the health, safety or personal rights to children in care.

- ADMISSION PROCEDURES AND PARENTAL AND AUTH REP RIGHTS- Upon presentation of identification, the responsible parent or guardian of a child receiving services in a child day care facility has the right to enter and inspect the facility without advance notice during the normal operating hours of the facility. The facility shall inform parents and guardians of children receiving services in the facility of the right of the parents and guardians to inspect the facility pursuant to this section. This requirement was not met as evidenced of staff interviews. Staff acknowledged that parents were denied entry into the building to mitigate the spread of COVID-19. This poses a potential risk to the health, safety or personal rights of children in care.
