Photographs obtained revealed two infants were swaddled in thick blankets, and placed to sleep on soft cushions next to each other while at the facility. This requirement was not met as evidenced by record review and interviews. There were inconsistencies with washing bottles at the facility, along with a picture of a moldy bottle prepared and used by an infant.
