(b) There shall be a ratio of one teacher for every four infants in attendance. This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above. During the investigation, it was determined that the facility had been operating out of ratio on multiple occasions. Evidence obtained revealed that on 04/01/2026, 04/20/2026, 04/22/2026, and 04/28/2026, the facility was operating out of ratio by having one staff caring for more than four infants. This poses an immediate risk to the health, safety, and personal rights of children in care.
