Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in... Supervision shall include visual observation.This requirement was not met as evidenced by: Based on video review, LPA Herrera observed child #1 was left with an absence of supervision for approximately 4 to 5 minutes in the infant outside play area, which poses an immediate risk to the health, safety or personal rights of children in care. An immediate $500 civil penalty is being assessed.
