Supervision shall include visual observation.This requirement was not met as evidenced by: Based on video review, LPA Herrera observed child #1 was left with an absence of supervision for approximately 4 to 5 minutes in the infant outside play area, which poses an immediate risk to the health, safety or personal rights of children in care.
