Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. LPA's observed play structure had paint peeling and had rust spots exposed on the climbing stairs. Swings did not have have cushioning material to absorb falls and top rail has rust which poses a potential health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above. During the inspection of the play yard for the school aged license, LPA Ocegueda observed that there was rusting and rubber cracking on the platform of the play structure. Also observed was a lack of cushioning material under the swing set and multiple holes and cracking on the cement area that caused potential tripping hazards. This poses/posed a potential health, safety or personal rights risk to persons in care.
