Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not met as evidenced by: Based on LPA observation, LPA observed in North and South classrooms multiple damaged suspended ceilings from water leaks and normal wear and tear. The licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care.

No child(ren) shall be left without supervision, including visual observation, of a teacher at any time. This requirement is not met as evidenced by: During the inspection, the LPA observed a child enter the bathroom by them self, and without anyone supervising. The child finished using the potty and washed their hands and then exited. The LPA had to point out to staff that the child was in the bathroom and no one noticed. This poses a potential health, safety or personal rights risk to persons in care.
