Fixtures, Furniture, Equipment and Supplies (n) Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analysts (LPAs) observed that the lining on the playground structure's stairs was broken and jagged in some areas, leaving sharp and pointed parts exposed.
Personnel Records (d) All personnel records shall be maintained at the child care center and shall be available to the licensing agency for review. This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above in that LPA observed that Staff 2, Staff 4, and Staff 5 did not have a personnel file on site. LPA observed that Staff 2 had taken the incorrect Mandated Reporter, and that Staff 4 and Staff 5 did not have a Mandated Reporter on file. LPA was informed that Staff do have a file, yet are located off site, which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) The center shall give each authorized representative a copy of the Personal Rights form (LIC 613A [9/96]). This requirement is not met as evidenced by: Based on record review, the licensee did not comply with the section cited above. During today's inspection, all children's files were missing signed LIC 613A. Lead teacher stated it was not given to parents which poses a potential health, safety or personal rights risk to persons in care.
