- Outdoor Activity Space (d) The surface of the outdoor activity space shall be maintained: (2) Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above in that LPA observed three wood panels located under the patio that had sharp nails, that area accessible to children in care. LPA also observed that there is a potential tripping hazard by the water hose located in the backyard, which poses/posed a potential health, safety or personal rights risk to persons in care.

- Responsibility for Providing Care and Supervision. No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by information obtained from the facility. On 5/16/23 facility self-reported that on 5/12/23 Child #1 was left unsupervised outside for no less than 30 minutes.
