(1) No child(ren) shall be left without the supervision of a teacher at any time…Supervision shall include visual observation. This requirement was not met as evidenced by: Based on interviews and records reviewed, staff did not ensure supervision of Child #1, who was mistakenly transported to an elementary school site and left unsupervised. This posed an immediate health and safety risk.
