101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs (1) No child(ren) shall be left without the supervision of a teacher at any time, Supervision shall include visual observation. This requirement is not met as evidenced by: Per investigation and information on 1/11/24 C1 was left alone unsupervised in the yard for at least 3-4 minutes. Class transitioned indoor with 2 staff who did not notice a child left behind. Later another staff found child. C1 stayed within the enclosed yard and premises. This is lack of supervision and posed a potential risk to health and safety of child in care
