101229 Responsibility for Providing Care and Supervision (a) The licensee shall provide care and supervision as necessary to meet the children's needs. THIS REQUIREMENT WAS NOT MET AS EVIDENCED BY : a tour of the facility, interviews and record reviews which revealed a 2 year old child ran onto a play structure, fell and injured themselves. Staff did not discourage the child from running onto the structure which is meant for climbing only.

Per director, after notification of the room being out of ratio, a staff member went into the room to bring the facility into compliance. Licensee will submit a written plan of action to ensure the proper ratios are kept at all times. Licensee will submit the plan of action by 7/25/24
