The licensee shall ensure that each child is accorded the following personal rights: To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature... This requirement is not met as evidenced by: Based on interviews and record review, the licensee did not comply with the section cited above in when C1 was pulling away from S1 his elbow was dislocated which poses/posed a immediate health, safety or personal rights risk to persons in care.

This requirement is not met as evidenced by: Based on interview and record review the school was notified 2/12/25 that C1 recieved medical treatment but did not inform licensing until 2/18/25, which poses a potential health and safety risk to children in care.

Director stated children used it for drinking water until March 2020 and not been used since then. Since children were exposed to water with lead, this posed a potential risk to Health and Safety.
