(1) No child(ren) shall be left without the supervision of a teacher at any time...Supervision shall include visual observation. This requirement is not met as evidenced by: A child was left behind in the bathroom after the classroom bathroom break.
(a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance... This requirement was not met as evidenced by: Based on observations and interviews, it was determined by LPA that on 4/16/2025 the facility was briefly out of ratio. This poses a potential risk to the health and safety of the children in care.
