This requirement was not met due to the preschool classroom had 13 children enrolled and attending, and at times the 13 preschool age children were supervised by 1 adult which poses a potential health and safety risk to children in care.

Buildings and Grounds. The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement is not being met as evidence by: LPA observed items within the children's bathrooms to have heavy equipment that has the potential to injure a child should it fall. Additionally, LPA observed cubbies within the "physical arena/gross motor skills classroom" to not be anchored.
