No child(ren) shall be left without the supervision of a teacher at any time, ..... Supervision shall include visual observation. This requirement is not being met as evidence by: Interviews conducted stated that an incident had occurred that while staff were present, they did not observe the incident occur which resulted in a child sustaining a deep scratch by another child.
